Linked Forms
Organisations can use the Public Survey feature to create Linked Forms whereby responses can be associated with Projects, Activities or specific Sessions.
This feature can be helpful to capture extra information in a structured way, which can then be associated back to these areas of an organisation's Upshot account.
Common examples of how organisations use this include:
- Session Evaluation Form
- Session / Activity / Project Plans
- Session Feedback Form
- Trip Plans
- Risk Assessments
Another common example may be capturing information that is currently recorded within the Session Notes free text box in a more structured and easily reportable way.
Creating the link
When creating the Public Survey users will be able to Add question with a:
- Project link
- Activity link
- Session link


Note: The survey can only contain one of these 'link' questions.
A suggestion for the wording of the question in the above example may be ‘Which session does this relate to?’
When presented with the question the respondent would see drop downs for each element that are auto populated from the list of Project, Activities and Sessions within your Upshot account.

Note: The question will display options for all Projects*, Activities or Sessions across an organisations Upshot account.
*For Projects, this will only display those whose end date is either in the future or less than approximately six months in the past.
The remaining questions on the survey would be added in the usual way, with the survey finally being Saved to be ready for use.
As with all Public Surveys to collect responses a deployment would be created by clicking Generate survey link and then shared with the appropriate audience. More about Public Surveys here.
Where to find the linked responses
Adding one of the linked questions into a Public Survey will then impact the results in two ways:
- Displaying the response within the linked area of the system
- Additional information within the survey results
Displaying the response within the linked area of the system
One of the key advantages of using this feature is that the survey response will now be displayed alongside the linked element. Once a response has been submitted, this will display the name of the survey and the name of the deployment.
- Project - Where Public Survey submissions are linked to a Project, these responses are shown within the General tab of the Project.

- Activities - Where Public Survey submissions are linked to an Activity these responses are shown within the Activity configuration page, viewable via Edit Activity.


- Sessions - Where Public Survey submissions are linked to a Session, these responses are shown within the specific session register.


Additional information within the survey results

Specifically for any linked forms, additional filters will show under the responses within the Survey deployment, allowing these to be filtered to a specific Project, Activity or Session as applicable:

Under each response the linked element will also show.
Once filtered this impacts any response bar charts for quantitative questions, only showing detail related to these responses.
Note: : The filter applied does not impact the downloaded results, this will continue to download all deployment responses.
Within the downloaded results extra information will be presented to the right-hand side of any survey results detailing associated information about the Project, Activity or Session as appropriate.

Example Use Cases
Use Case 1 - Delivery Organisation
A Delivery Organisation looks to collect extra feedback from the individual that runs the session (Coach, Tutor, Session Leader, Volunteer). This can be general feedback of what worked well and what did not at the session, alongside additional information such as quotes heard from participants as well as any other comments.
Creating a Public Survey with the Session Link question can effectively create a Session Evaluation form in a structured way. The individual that ran the session can complete this following the session and this additional feedback then sits alongside the register within the system.
When analysing all the responses to this survey it is also clear which response relates to which session either within the responses or the downloaded results.
Use Case 2 - Facilitating Organisation
A Facilitating Organisation might want to collect information from Delivery Organisation staff on how a project went, or how certain activities were delivered.
Creating a Public Survey, with the Project Link question, can ensure that Delivery Organisation staff select the project from a drop down when responding that is consistent with the terminology recognised by the Facilitating Organisation, rather than creating their own free text label for this.
